SNAP Survey

This survey is intended for people living in Minnesota who are receiving or seeking support to meet their food needs.

If this describes you, please continue so we may better understand your experience and explore how to make programs like SNAP (food stamps) work better.

Please be sure to complete the survey by Monday, November 16 in order to be eligible for a $25 gift card. Limit one gift certificate per household.

The printable paper survey is available by request in English, Spanish, Somali, Hmong, Karen, and Russian.
